[topicmapmail] XML Schema to XTM

I started doing one of those - its not straightforward at all given
XSD's syntax. I didn't get round to finishing it off though...

FYI: I made better progress using the output from XSV. Although that
output format is not documented (AFAIK), it does remove the need to do
datatype processing in your stylesheet.
